a) Who you are and what academic bodies or organisations you're affiliated with
b) What the purpose of the survey is
c) The methodologies chosen for selecting respondents and phrasing questions to eliminate bias, prevent sample skew through self-selection, etc
d) Where the results will be reported, what they'll be used for and by whom?

I can see you're offering a random prize - not wishing to be all ironic, given the subject matter, but can you explain what transparency there will be in the selection of winners, etc? ;-)
